Information Change Form

Use this form to make CHANGES to a current application for undergraduate admission.

PLEASE DO NOT USE THIS FORM IF YOU HAVE NOT ALREADY APPLIED.

Please enter your name, Social Security number and date of birth, PLUS THE NECESSARY CHANGE(S). It is not necessary to fill out fields that you are not changing.

New Applicants: Please do not submit this form until you have mailed us the Application Data Verification form we send you in response to receiving your application.

Transfer Students: Please send any transcripts for coursework you have completed since your application was submitted to the Office of Admissions.

Previous Applicants: If you have previously applied for admission to UGA, did not enroll and would like us to update your application information to a future term, YOU MUST NOT USE THIS FORM. Instead, you must submit a completed 2005-06 application with all the supporting documentation and appropriate fees.
